High rates of paediatric medication errors.
A paper in Archives of Disease in Childhood by Ghaleb et al, reports very high rates of paediatric drug prescibing and administration errors in 5 London hospitals. The research found that 13% of medication orders contained errors and 19% of drug administrations were potentially erroneous.  There was considerable variation in the potential seriousness of the errors and the study did not formally assess the clinical significance of the errors.  However, this large study highlights that errors are not uncommon and will hopefully lead to strategies to improve prescribing and administration of drugs to children and young people.
